Root Cause Analysis
Description
Effective problem solving to prevent problem recurrence requires a thorough investigation and identification of the true root cause of the problem. This course will teach the basics of root cause analysis and corrective action using industry proven tools and techniques including:
- Problem Statement – understanding and defining the problem
- Identification and confirmation of direct, contributing, and root causes.
- Identification, selection, validation, and implementation of solutions.
- Monitoring effectiveness of solutions.
Objectives
- Learn what Root Cause Analysis and Correction Action (RCCA) is: Learn how to identify and address any problem to ensure effective, fact-based solutions
- Learn how to break a problem down to identify all possible conditions, then to isolate the real cause that needs to be addressed
- Learn what Corrective Action is, and how to identify solutions that prevent recurrence
- Become familiar with graphical problem solving techniques such as Fault Tree Analyses, Design of Experiments, Nominal Group Technique, Failure Mode and Effect Analysis
